What is the Wellness Assessment worksheet?
The wellness assessment worksheet is a two-page self-rating grid running from Physical Wellness through to Spiritual Wellness. Fifteen statements, three in each area, are rated 1 for Strongly Disagree up to 5 for Strongly Agree. Two written prompts close the sheet, on the area that feels most satisfying and the one needing most improvement.
How to use the Wellness Assessment worksheet
Use this sheet in session and total it in the room. Work down one area at a time, since ratings flatten once a client reads ahead across the whole grid. The Spiritual Wellness rows are the ones most often skipped, so say early that a low rating there is information rather than a failing. Your clients can rate the same sheet again a month later, where movement shows more clearly than in a single sitting.
