What is the Trauma Symptoms Checklist worksheet?

Each section lists five symptoms, leaves two blank Other lines, and ends with a written question asking which one affects the client most.

How to use the Trauma Symptoms Checklist worksheet

Use this sheet in session, so you can pace the checking and stop when arousal climbs. Start with Emotional Symptoms, since clients name feelings sooner than they name behavior. Leave Thought Patterns until last, because the five beliefs printed there usually open a longer conversation than a check mark closes. Your clients can answer the written question under each section aloud while you record the wording they use.
