The therapy goals worksheet moves a client from a stated reason for coming to two goals written in SMART form. It opens with two questions: ‘What is your main reason for seeking therapy?’ and ‘What specific changes would you like to see?’. Page two then leaves Goal #1 and Goal #2 blank.
How to use the Therapy Goals worksheet
Use this sheet both in session and as homework. Do the two opening questions in session, since a client’s first answer is usually a complaint about someone else rather than a goal.
