  • Stay motivated and engaged in therapy.
  • Track progress and celebrate achievements.
  • Stay accountable to the therapeutic process

What is the Therapy Goals worksheet?

The therapy goals worksheet moves a client from a stated reason for coming to two goals written in SMART form. It opens with two questions: ‘What is your main reason for seeking therapy?’ and ‘What specific changes would you like to see?’. Page two then leaves Goal #1 and Goal #2 blank.

How to use the Therapy Goals worksheet

Use this sheet both in session and as homework. Do the two opening questions in session, since a client’s first answer is usually a complaint about someone else rather than a goal. Setting SMART Goals then gives the worked anxiety example to translate. Your clients can draft Goal #1 alone, but read the Measurable and Time-bound lines yourself before the next session. Most drafts fail there and stay intentions.

The download is two printable pages, not fillable. Page one holds Identifying Your Therapy Goals with its two questions, followed by a short account of what SMART stands for. One worked example builds on ‘I want to manage my anxiety better’. Page two is the working space: Goal #1 and Goal #2, each under the five SMART lines spelled out one at a time.
