What is the Relapse Prevention Plan worksheet?

The relapse prevention plan worksheet is a two-page plan that a client fills in rather than reads. Page one pairs each Trigger with the Coping Strategies that answer it and lists Your Support Network. Page two is a Daily Maintenance Plan split into morning, afternoon and night.

How to use the Relapse Prevention Plan worksheet

Use this plan both in session and as homework. Build the Identify your Triggers and Coping Strategies grid in session, one row at a time, so each trigger gets a strategy the client has used before. Name the people in Your Support Network with the client present, since a list written alone tends to be shorter. Your clients can draft the Morning, Afternoon and Night Routine boxes between sessions and report what held.
