
What is the Fear Ladder worksheet?
The fear ladder pdf is a one-page grid with five numbered steps, running from step 1 at the bottom to step 5 at the top. Each step pairs a Situation with an Anxiety Level rated 1 to 10. Two prompts above the grid ask What is your fear? and What is your goal?
How to use the Fear Ladder worksheet
Use this sheet both in session and as homework. Build the ladder in session, starting at step 1 with a situation that causes only mild anxiety and working up. Rate each entry on the Anxiety Level scale before the client attempts it, and rate it again afterwards. Your clients can repeat a step until the rating drops rather than climbing on schedule. If a client jumps to the top step first, the ladder stops being graded exposure and becomes a single hard test.
